QUALIFYING DOCUMENTS

DS-DE 9 – Appointment of Campaign Treasurer & Designation of Campaign Depository for Candidates  Required by Florida Statutes.                This form must be filed with the City Clerk’s office before opening a campaign account. The Election Fee must be paid from this account.

Notice of Candidacy – Please print name as you wish it to appear on the ballot.

DS-DE 25 Loyalty Oath - Required by Florida Statutes. City Clerk administers the oath.

DS-DE 84 Statement of Candidate - Required by Florida Statutes. This form must be filed with the City Clerk’s office by the candidate within 10 days after Appointment of  Campaign Treasurer form is filed.

CE Form 1 – Financial Disclosure  Required by Florida Statutes.

Affidavit of Undue Burden – Required by Florida Statutes.  The Clerk’s office will notarize this form for the candidates

Logistic and Accuracy Test NoticeLegal Notice – written notice of election equipment testing dates
